ПЕРИОДИЧЕСКАЯ БОЛЕЗНЬ: СЛУЧАЙ ДОСТИЖЕНИЯ РЕМИССИИ У ПАЦИЕНТА С ПОМОЩЬЮ МЕТОДОВ ПЛАЗМАФЕРЕЗА И ВНУТРИВЕННОГО ЛАЗЕРНОГО ОБЛУЧЕНИЯ КРОВИ

Резюме: Семейная средиземноморская лихорадка (ССЛ), (наследственный семейный амилоидоз) является одним из врожденных моногенных аутовоспалительных заболеваний. Причиной его развития является наследственный дефект различных участков гена MEFV, в результате чего изменяется структура белка пирина, который утрачивает регуляторное влияние на инфламмасомы клеток, что в конечном итоге приводит к гибели иммунных клеток – пироптозу, сопровождаемому местным и системным воспалительным ответом. Представлен пример успешного лечения пациента с обострением семейной средиземноморской лихорадки после военной травмы, имеющего ограничения к патогенетической терапии колхицином, которому для купирования воспалительной реакции проводили сеансы плазмафереза (ПФ) и внутривенного лазерного облучения крови (ВЛОК). Заключение. Подтвердили эффективность и безопасность применения плазмафереза (ПФ) и фотогемотерапии (ФГТ) в комплексе лечения пациента с обострением ССЛ, заболевания моногенной аутовоспалительной природы, при недоступности или неэф,фективности базисной терапии колхицином.

EXTRACORPOREAL HEMOCORRECTION AND PHOTOHEMOTHERAPY AS COMPONENTS OF THE TREATMENT OF EXACERBATION OF FAMILIAL MEDITERRANEAN FEVER IN A PATIENT WITH SEVERE MILITARY TRAUMA

Summary: Familial Mediterranean fever is one of the congenital monogenic autoinflammatory diseases. In the absence of adequate treatment of exacerbations, a characteristic feature of familial Mediterranean fever is the development of organ dysfunction, mainly associated with the development of AA amyloidosis. The cause of the disease is a hereditary defect in various parts of the MEFV gene; as a result, the structure of the pyrin protein changes, which loses its regulatory effect on cell inflammasomes, which ultimately leads to the death of immune cells — pyroptosis. Exacerbations of the disease can occur either spontaneously or as a result of activation of a wide range of triggers: exposure to cold, eating foods rich in fat, heavy physical activity, previous surgical operations, infections, etc. The key factors influencing the prognosis and quality of life of patients are the frequency, severity and duration of periods of exacerbation of the disease. The main treatment for familial Mediterranean fever is colchicine. If ineffective, anticytokine and cytostatic drugs are prescribed. This paper presents a clinical case of the use of plasmapheresis and intravenous laser irradiation of blood in the treatment of exacerbation of familial Mediterranean fever in a patient who has restrictions on the use of basic and reserve therapy due to polytrauma, with a satisfactory clinical and laboratory effect and the absence of reactions and complications.
